Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM® or ACM™ certification upon entry into this position OR within two years of entry into the position.
CCM® or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.

At least 2 years experience working with underserved or special needs populations with varied health, economic and educational circumstances, and experience with affordable housing and/or related community resources/social services, or equivalent combination of relevant education and experience.
Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements.
Knowledge of applicable state and federal guidelines/regulations (e.g., the fair housing act and the rights of people with disabilities under section 504 of the rehabilitation act of 1973).
Ability to prioritize and manage multiple deadlines.
