- Collaborate with healthcare teams to ensure coordinated care
- Support discharge planning and facilitate continuity of care as needed
- Completion of accredited PTA program
- Completion of National Physical Therapy Exam (NPTE)
- KS PTA licensure/certification
- First Aid Certifications (BLS) strongly recommended
- Pay: $1,315.62 - $1,356.31 / week
- Location: Downs, KS
- Assignment Length: 12
- Shift: Day - up to 40.00 hours per week
- Setting: Skilled Nursing Facility - SNF
