- Maintain accurate documentation of patient sessions and progress
- Collaborate with healthcare teams to ensure coordinated care
- Support discharge planning and facilitate continuity of care as needed
- Completion of accredited PTA program
- Completion of National Physical Therapy Exam (NPTE)
- MA PTA licensure/certification
- First Aid Certifications (BLS) strongly recommended
- Pay: $1,242.45 - $1,280.88 / week
- Location: Agawam, MA
- Assignment Length: 254
- Shift: Day - up to 40.00 hours per week
- Setting: Skilled Nursing Facility - SNF
- Workplace: On-site
