- Conduct thorough PT evaluations and develop individualized treatment plans
- Deliver evidence-based therapeutic interventions to improve mobility, strength, and functional independence
- Document all patient care in accordance with state, federal, and facility guidelines
- Collaborate with nursing, therapy, and medical teams to ensure coordinated care
- Educate residents and family members on exercise programs, safety, and functional strategies
- Participate in care conferences and contribute to ongoing plan-of-care updates
- Valid New Mexico state license as a Physical Therapist (PT)
- Degree in Physical Therapy from an accredited program
- No experience with skilled nursing rehabilitation, supportive team to mentor and guide
- Strong communication skills and a passion for patient-centered care
- Competitive starting rate of $60.00/hour
