- Location: Charlotte, NC (Zip: 28273) - covering Mecklenburg County and Union County
- Assignment Dates: January 12, 2026 - April 11, 2026
- Employment Type: Travel LPN / Travel Home Health Nurse
- Healthcare Setting: Homecare / Home Health
- Shift: 8-hour day shifts, Monday-Friday
- Shifts per Week: 5
- Hours per Week: 40
- Estimated Weekly Pay: $1,460 - $1,700
- Mileage Reimbursement: $0.41/mile
- Active LPN (Licensed Practical Nurse) license in North Carolina or a Compact State (NC LPN license preferred)
- Minimum 1 year of LPN clinical experience required; home health / homecare experience strongly preferred
- Clinical skills in wound care, IV therapy, port care, and wound vac management
- Experience with electronic documentation (HomeCare HomeBase / HCHB preferred)
- Ability to rotate on-call weekends/evenings as needed
- Strong communication, assessment, and documentation skills
- Valid driver's license and reliable transportation required for travel LPN duties in Charlotte and surrounding counties
- Provide patient-centered homecare nursing according to individualized care plans while promoting dignity and independence
- Perform skilled nursing tasks: monitoring vitals, administering medications, enteral feedings, oxygen therapy, ostomy care, and wound treatments
- Provide IV and port care per agency policy and report changes in patient condition promptly
- Collaborate with interdisciplinary team members including PT, OT, and SLP to coordinate patient care
- Maintain infection control and report communicable diseases or infection concerns
- Complete accurate and timely electronic documentation of nursing actions, patient progress, and outcomes (HCHB experience a plus)
- Manage supplies, medications, and equipment inventory for assigned caseload
- Report incidents, complete incident reports, and follow agency escalation processes
- Identify and report faulty medical equipment and coordinate timely repairs
- Competitive travel LPN pay: estimated $1,460 - $1,700 per week
- Mileage reimbursement: $0.41/mile
