Position Summary We are seeking a compassionate and skilled Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis. In this role, you will evaluate and treat residents, develop individualized treatment plans, collaborate with interdisciplinary team members, and help residents achieve their highest level of independence and quality of life. Key Responsibilities
Evaluate residents and develop individualized treatment plans
Provide occupational therapy services in accordance with physician orders and established goals
Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely
Monitor resident progress and modify treatment plans as appropriate
Communicate with physicians, nursing staff, residents, families, and other healthcare professionals regarding treatment plans and outcomes
Participate in care conferences, interdisciplinary meetings, and discharge planning
Educate residents, families, and caregivers on therapy goals and home programs
Conduct in-service training and education for facility staff as needed
Ensure compliance with state practice acts, facility policies, and regulatory requirements
Support residents in achieving their highest level of function, independence, and quality of life
QualificationsEducation and Experience
Bachelor's degree in Occupational Therapy required
Master's or Doctorate degree in Occupational Therapy preferred
Experience in skilled nursing, rehabilitation, or long-term care preferred
Experience with Casamba and PointClickCare (PCC) preferred
Licenses and Certifications
Current Occupational Therapist (OTR) license in South Carolina in good standing
Must maintain all required continuing education requirements
Must maintain licensure in good standing with the State Board
