Description

Position Summary

The PT provides intermittent physical therapy in patients' homes, following physician orders and under the supervision of the Director of Nursing.

Deliver therapy services to restore physical function, reduce pathology, promote health, and support patients in achieving productive living, as prescribed by physicians.

Essential Duties and Responsibilities

  • Conduct initial evaluations and develop treatment plans.
  • Administer prescribed physical therapy and monitor progress.
  • Write clinical notes, reevaluations, and discharge summaries.
  • Collaborate with physicians, families, and healthcare teams.
  • Supervise assistants and participate in staff training and audits.
  • Ensure compliance with agency policies and healthcare regulations.
  • Maintain accurate documentation and participate in performance improvement activities.

Knowledge, Skills and Abilities

  • Strong communication, supervisory abilities, and independent judgment.
  • Must meet physical demands (e.g., standing, lifting).
  • Ability to work in stressful environments with professionalism.
  • Must adhere to dress code and hygiene standards.

Education & Experience

  • Preferably 2 years in home/community service.
  • Graduate of an accredited Physical Therapy program; licensed in Texas.
