Overview Our Visiting Licensed Practical Nurse for our VNA Certified division provides short- and long-term home care services for patients covered by Medicare, Medicaid, Private Insurance, and Private Pay. Services include Skilled Nursing, Physical Therapy,Occupational Therapy, and Home Health Aide support delivered in patients' homes. We serve diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs. Newly hired Variable Hour and Part Time employees are measured for eligibility into our ACA Benefits plans over their first twelve months of employment (also known as the Initial Measurement Period). Deliver quality patient care in compliance with physician orders under direction of their Registered Nurse teammate.
Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers.
Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
Educate clients and their family members based on client's specific needs.
Provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.
Qualifications
An active Nursing License in the state of application
Current CPR certification (with hands-on component) or able to obtain one
Current TB test
Must have drivers license, reliable transportation, and auto insurance
Preferred
Medicare Skilled Nursing experience
1-year LPN experience in a health care setting

Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
Compact licenses must be transferred to your state of residence within 90 days
Current CPR certification (with hands-on component)
TB skin test (current within last 12 months)
Six months prior hands-on nursing experience preferred but not required
Must have reliable transportation
