Entitlements Case Manager

This role can be fulfilled either as a W-2 employee or by providing services as an independent contractor (1099). Candidates must select their preferred engagement type upon applying. The details below outline both the 1099 independent contractor and W-2 employee versions of this opportunity. Client Service Location Expectations: Regardless of which option you choose below, the majority of assigned clients will reside in community-based group homes. Clients who do not reside in a group home may be seen in your assigned community-based office, at the company's main office, or via telehealthprovided that either you or the client is physically located at one of these approved sites during the service.

Caseload Makeup:

  • 11 Category A clients to be serviced at community-based meeting room 1620 Druid Hill Avenue, Baltimore, MD 21217
  • 14 Category A clients to be serviced at community-based meeting room 1723 Gwynns Falls Parkway, Baltimore, MD 21217
  • 4 other than Category A clients to be serviced in the community or at 3310 Eastern Avenue, Baltimore, MD 21224

Option 1: 1099 Independent Contractor Version

Scope of Service: Entitlements Case Manager Independent Contractor

CLASSIFICATION: Part-Time Contract Position (1099). Fee-for-service.

Lighthouse Behavioral Health Center, LLC (LBHC): Part-time, 1099 contractor Approximately 20 hours per week (50% of total weekly hours)

Community Wellness Outpatient Mental Health Center, LLC (CWOMHC): Part-time, 1099 contractor Approximately 20 hours per week (50% of total weekly hours)

DISCLOSURE MULTI-ENTITY CONTRACTOR ROLE: This 1099 Independent Contractor role for a Entitlements Case Manager includes responsibilities that span across the following two organizations, all operating under shared leadership:

  • FOCUS #1: Lighthouse Behavioral Health Center, LLC
  • FOCUS #2: Community Wellness Outpatient Mental Health Center, LLC
  • FOCUS #3: Community Wellness Psychiatric Rehabilitation Program

As an independent contractor, you will maintain separate contractual agreements with each organization. You will invoice and receive compensation separately from each entity based on the time worked and services delivered for their respective programs. This means you are not an employee of any of the entities and are instead engaged as a self-employed contractor for each. Though the contractor is paid on a fee for service basis and not an hourly basis, the services will require approximately 40 hours per week in to render assigned services as scheduled over 5 days, with an estimated one-half of the services rendered allocated to each organization. While day-to-day scheduling and care coordination may be managed collaboratively to support continuity across programs, each organization retains responsibility for its own scope of work, compliance requirements, and payment processes. As a 1099 contractor, you are responsible for your own taxes, benefits, and liability coverage, and you must adhere to all contractual terms and professional licensing requirements relevant to each organization.

Overview Generally, assigned clients will reside in community based group homes. Assigned clients who do not reside in group homes may receive services in your assigned community based office, at the company's main office, or via telehealth if you or the client are in one of these locations.

We are seeking a qualified and licensed Entitlements Case Manager to provide case management services on an independent contractor basis. The contractor will schedule and facilitate group and individual case management sessions, conduct case management assessments, and complete client documentation within the designated service period.

Option 2: W-2 Employment Version

CLASSIFICATION: Part-Time (W2). Fee-for-service.

Lighthouse Behavioral Health Center, LLC (LBHC): Part-time, W-2 employee Approximately 20 hours per week (50% of total weekly hours)

Community Wellness Outpatient Mental Health Center, LLC (CWOMHC): Part-time, W-2 employee Approximately 20 hours per week (50% of total weekly hours)

BENEFITS PACKAGE: Paid time off; health, dental, and vision insurance reimbursement; professional development support; retirement plan; supplemental disability insurance

WORK SCHEDULE: 45 days per week, typically MondayFriday, 8:00 AM5:00 PM

COMPENSATION: $50,000$100,000 annually (fee-for-service, commensurate with experience and credentials)

TRAVEL: Up to 10% (interoffice and site-based)

UNSCHEDULED DUTIES & RESPONSIBILITIES

  • Support supervisor with work-related tasks and serve as program POC as needed
  • Comply with all COMAR, CARF, and company policies; assist in new hire orientation as assigned
  • Maintain client confidentiality and safety; participate in required trainings and drills
  • Contribute to internal/external audits and quality improvement activities
  • Ensure documents are reviewed/co-signed by the Medical Director or Clinical Supervisor when required

PHYSICAL DEMANDS & WORKING CONDITIONS

  • Regular walking, standing, and light lifting (up to 10 lbs.); must drive as needed
  • Work in office or community settings, with occasional exposure to infectious diseases or volatile situations

COMPETENCIES & SKILLS

  • 12 years' experience with dual-diagnosis or high-acuity populations preferred
  • Knowledge of Medicaid documentation, CARF standards, and trauma-informed care
  • Strong understanding of mental health and substance use disorders (DSM-5-TR)
  • Skilled in creating measurable, recovery-oriented goals
  • Excellent documentation, time management, and communication skills
  • Ability to work both independently and collaboratively
  • Commitment to equity, inclusion, and culturally responsive care

EDUCATION / TRAINING / QUALIFICATIONS

  • Education: Bachelor's degree in Psychology, Social Work, Counseling, or related field (Master's preferred).
  • Licensure/Certification (Maryland): LCADC, LGADC, CAC-AD, CSCAD, ADT, LCPC, LGPC, LCSW-C, LCSW, LMSW, LBSW, LCMFT, LGMFT, Psychologist, Registered Psychology Associate, CRNP-PMH, APRN-PMH, CNS-PMH, or eligible student intern in one of these disciplines.
