This home health Speech-Language Pathologist position is based in Honolulu, Hawaii, and serves patients across the island of Oahu. Depending on agency needs, you may also see occasional pediatric or young adult cases. The focus is on functional, patient-centered care that supports safety, independence, and quality of life within the home environment.Your responsibilities will include completing comprehensive speech, language, cognitive-communication, and swallowing evaluations; developing individualized plans of care; setting functional goals in collaboration with patients and families; and delivering skilled interventions that promote communication and swallowing safety. You will educate patients, caregivers, and family members on strategies, home exercise programs, and environmental modifications, and you will coordinate with interdisciplinary team members to ensure cohesive, high-quality care.A typical day involves traveling between patient homes across the Honolulu area and broader Oahu region, managing a caseload that allows for focused, unhurried one-to-one care. Patient visit frequency and daily visit volumes are structured to support high-quality documentation, communication with the care team, and appropriate travel time between homes. You will collaborate closely with nursing, physical therapy, occupational therapy, and other disciplines to support holistic, integrated home health services.You will use an electronic medical record system to document evaluations, daily notes, progress updates, and discharge summaries, with support from office and clinical leadership to ensure you have the tools needed to be efficient and effective in the field. Orientation and ongoing professional support are provided to help you become comfortable with the service area, workflow, and documentation requirements.Shifts are typically daytime, weekday hours, with some flexibility based on patient needs and agency scheduling practices. Weekend or occasional on-call coverage may be requested, depending on census and staffing patterns, but the overall schedule is designed to support work-life balance.
