Description

The Registered Nurse (RN) provides patient-centered nursing care within a community healthsetting at SDCHC. Working collaboratively with an interdisciplinary team, the RN supports clinic operations through patient assessment, triage, treatment, and education, with a focus on preventive care and chronic disease management. The RN ensures compliance with all regulatory requirements and maintains accurate documentation in the electronic health record (EHR) while promoting high-quality, coordinated care.
Essential Duties and Responsibilities:
Patient Care

  1. Performs appropriate treatments as ordered by physician in an accurate and timely manner.
  2. Assess, plan, and implement patient care in accordance with SDCHC standards, policies, and procedures.
  3. Conduct triage, manage chronic conditions, and administer medications or treatments as prescribed.
  4. Provide education and counseling to patients on various health conditions and preventive care measures.
  5. Assist in minor procedures and perform wound care, immunizations, and diagnostic testing (e.g., blood pressure, glucose levels).
  6. Support the care of patients with chronic illnesses (e.g., diabetes, hypertension) through case management.
  7. Performs therapeutic nursing interventions as established by individualized plan of care for the patient and his/her family.
  8. Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocols.
Patient Care (continue)
  1. May assist clinic leadership manager with disciplinary actions, and performance evaluations.
  2. Checks and oversees maintenance of clinic logs including refrigerator and freezer temperatures twice daily (AM/PM), room cleaning, hazardous waste, and expiration dates.
  3. Assists/oversees medical supplies inventory and materials.
  4. Ensures company policy is being adhered to by MA's, take corrective action when necessary.
  5. Assists clinic manager with HEDIS, UDS, GPRA measures.
  6. Assists clinic manager in facilitating inter-department communication.
  7. Performs other duties as assigned.
Care Coordination
  1. Collaborate with providers and clinical staff to coordinate patient care, including follow-up appointments and referrals.
  2. Participate in care team meetings to ensure quality and continuity of care.
  3. Provide patient education regarding medications, treatments, and health maintenance.
Health Screenings and Preventive Services
  1. Participate in community outreach efforts to promote health education and preventive care.
  2. Administer vaccines and screenings per federal, state, and organizational guidelines.
Documentation and Compliance
  1. Maintain accurate and timely documentation in the Electronic Health Record (EHR) system.
  2. Ensure compliance with FQHC standards, HIPAA, OSHA, and infection control policies.
  3. Participate in quality improvement initiatives to enhance patient care and outcomes.
  4. Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
  5. Demonstrates proficiency in the use of the clinic's E.H.R, including appropriate documentation of visits, telephone notes, medical Rx refill requests, referral process, and data entry into patient's chart.
  6. Checks CAIR for up-to-date IZ records.
Patient Advocacy
  1. Act as an advocate for patients, ensuring their rights are respected and their health needs are met.
  2. Provide culturally sensitive care that recognizes the unique needs of underserved and vulnerable populations.
Qualifications
Education:
  1. Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program required.
  2. Bachelor of Science in Nursing (BSN) preferred.
  3. Education must meet all applicable state and federal requirements for licensure as a Registered Nurse.
Licensure/Certification:
  1. Current, active, and unrestricted Registered Nurse (RN) license in the State of California required.
  2. Must maintain licensure in good standing throughout employment.
  3. Any restrictions, suspensions, or disciplinary actions must be reported immediately in accordance with organizational policy.
Experience:
  1. Minimum of one (1) year of clinical nursing experience preferred; experience in a community health center, ambulatory care, or Federally Qualified Health Center (FQHC) setting highly desirable.
  2. Experience working with diverse, underserved, or vulnerable populations preferred.
  3. Familiarity with electronic health record (EHR) systems (e.g., eClinicalWorks) preferred).
Special Conditions of Employment:
  1. CPR/ BLS certification: Maintain a current Basic Life Support (BLS) certification issued by the American Heart Association (AHA), the American Red Cross, or an equivalent organization. Certification must include an in-person, hands-on skills assessment. Online-only certifications are not accepted.
  2. Annual background checks: Consent to annual background checks as a condition of continued employment, to ensure compliance with organizational standards and eligibility requirements.
  3. For-Cause Drug Screening: Comply with drug screening requirements when initiated by the organization for cause, to support a safe, compliant, and drug-free workplace (including marijuana).
  4. Ongoing Compliance Requirements: Maintain up-to-date compliance with all required annual renewals, including professional licenses, certifications, physical examinations, TB testing, and mandatory regulatory trainings as assigned by the San Diego Community Health Center (SDCHC).
Work Environment
This job operates in a clinical setting, and the RN will be exposed to patients, bodily fluids, and communicable diseases. The RN may need to lift and support patients and work in a fast-paced environment. The role may also involve participating in community outreach programs or events.
