Physical Therapist

Overview Active In-Home Therapy is looking for a skilled and dedicated Physical Therapist to join our team. This is a PRN position (not full-time), meaning you'll work with us on an as-needed basis while enjoying flexible scheduling. You'll be hired as a W-2 employee,not as an independent contractor. This role is an excellent option if you're looking to supplement your current work while making a meaningful difference in patients' lives.

What You'll Do

  • Conduct thorough assessments of patients' physical conditions to determine rehabilitation needs.
  • Develop and implement individualized treatment plans in collaboration with the healthcare team.
  • Provide hands-on physical therapy interventions to improve strength, flexibility, balance, and coordination.
  • Educate patients and their families on therapeutic exercises, mobility aids, and safety strategies to promote independence.
  • Monitor and document patients' progress, adjusting plans as needed.
  • Communicate effectively with patients, families, and team members to ensure a positive and supportive care experience.

What We're Looking For

  • Bachelor's or Master's degree in Physical Therapy from an accredited program.
  • Current California Physical Therapist license.
  • Home health experience preferred but not required.
  • Strong clinical judgment, organization, and communication skills.
  • Reliable transportation for patient visits.
