Greenfield Rehabilitation Agency is excited to announce an opportunity for a Physical Therapist (PT) to join our therapy team at our newly acquired senior community in Sauk City, WI.
This role offers flexibility for full-time, part-time, or PRN schedules-a great opportunity for a dedicated and compassionate clinician to make a meaningful difference in the lives of residents while working in a supportive, therapist-led environment.

Your Responsibilities
  • Provide compassionate and effective physical therapy services to adult and geriatric residents
  • Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
  • Support residents in achieving personal goals and rehabilitation milestones
  • Accurately document care and maintain compliance standards (experience with Net Health a plus)
  • Collaborate with therapy and nursing teams to ensure patient-centered outcomes
  • Educate residents and families on safe techniques and strategies to maximize independence
  • Participate in team meetings and contribute to care planning
What You Bring
  • Current Wisconsin PT license
  • Strong clinical skills and a passion for working with older adults
  • Excellent communication and teamwork abilities
  • Flexibility, professionalism, and a positive attitude
