- Respiratory Therapist skills checklist [REQUIRED][DOCUMENT]
- 1 Supervisory Signed Reference, from the past year of employment [REQUIRED]
- Active American Heart Association BLS - [REQUIRED][DOCUMENT]
- Active NBRC - [REQUIRED][DOCUMENT]
- NH RCP Licensure. Will accept temporary. MUST be willing to apply for temp NH RCP at time of offer (7-14 day turnaround)
- License verification [REQUIRED][DOCUMENT]
- All licenses/certs must be current at time of submission
- Permanent address must be included in submission file [REQUIRED]
- RTO - must be included in submission. RTO submitted after offer may not be accommodated.
