Wellness Coach

The Wellness Coach supports patient wellness and health integration in outpatient clinics. Wellness coaches are an integral part of a patient's Care Team and work within multidisciplinary teams and other support staff. This position is responsible for engagement of clinic patients into a variety of healthcare activities through population-based assessments and screenings and tracking patients' follow-up and progress. Wellness coaches focus on prevention, health education, brief interventions and referral to additional care when indicated.

Salary: Starting pay is $20.00 - $22.05 hourly

Qualifications: The ideal candidate for the Wellness Coach position will have their high school degree (or equivalent). A bachelor's degree in social work, Psychology, Counseling, or equivalent field is preferred. Two years of volunteer or paid experience in a healthcare or social service setting is also preferred.
