Description

RN - Care Manager

Join a dedicated healthcare team as a Registered Nurse Care Manager, providing comprehensive care coordination for adult patients across an interdisciplinary environment. You will develop, monitor, and evaluate individualized care plans while maintaining respectful and effective communication with patients, families, and healthcare staff.

Essential Functions

  • Conduct needs assessments and identify care options and services.
  • Manage patient data related to case management, care planning, and quality improvement analysis.
  • Monitor care delivery through patient rounds, documentation, and communication with utilization review staff.
  • Perform utilization reviews for all patients and assist nursing staff with patient care as needed.
  • Support rehabilitation training and education for staff when required.
  • Ensure compliance with healthcare regulations, including HIPAA privacy and security standards.
  • Perform other duties as assigned by leadership.

Minimum Knowledge and Skills

  • Ability to develop patient care plans and communicate effectively.
  • Strong critical thinking skills to assess patient care needs.

Education and Experience

  • Completion of an accredited nursing program.
  • At least one year of experience as a registered nurse.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • Experience in care management.

Licensure and Certification

  • Current RN licensure in the relevant state.
  • American Heart Association Basic Life Support (BLS) certification preferred.
