Home Health - Physical Therapist

Key Responsibilities for Home Health Physical Therapist:

• Patient Assessments: Conduct evaluations of patients' physical abilities and limitations within their home setting, designing customized plans to enhance mobility, strength, and functional capabilities.
• Therapy Planning & Implementation: Develop and apply therapeutic programs to improve movement, balance, and daily function, focusing on the patient's goals and needs.
• Patient & Caregiver Education: Provide guidance to patients and their families on exercises, home modifications, and techniques to increase safety and mobility.
• Collaboration with Healthcare Team: Partner with physicians, nurses, occupational therapists, and other healthcare professionals to provide coordinated care tailored to each patient.
• Documentation: Maintain accurate and timely records of treatment plans, patient progress, and outcomes, in compliance with home health standards and regulations.

Qualifications for Home Health Physical Therapist:

• Education: Graduate of an accredited Physical Therapy program.
• Certification: Valid state licensure as a Physical Therapist (PT).
