Join a dynamic healthcare team in the Midwest region as a Clinical Pharmacist specializing in critical care. In this role, you will collaborate closely with physicians and other healthcare providers to optimize patient outcomes through expert pharmaceutical care.
Essential Responsibilities

  • Provide pharmaceutical care to patients ranging from newborns to geriatrics.
  • Complete, verify, and sign required documentation in compliance with legal and policy standards.
  • Participate in ongoing training and professional development activities.
  • Review patient medical histories and symptomology to recommend appropriate pharmaceutical therapies.
  • Develop individualized medication care plans in partnership with physicians and nursing staff, aligned with patient care goals.
  • Ensure thorough and accurate patient records, including past and current prescriptions, to identify potential drug interactions or contraindications.
  • Educate healthcare providers on emerging pharmaceuticals and their proper use.
  • Adhere to privacy and security regulations concerning Protected Health Information (PHI).
  • Maintain effective communication and professional relationships with patients, healthcare teams, contractors, and external agencies.
Education and Experience
  • Doctor of Pharmacy (Pharm.D.) from an ACPE-accredited School of Pharmacy or Bachelor of Science in Pharmacy.
  • Board Certification in Critical Care Pharmacy (BCCCP) is required.
  • Additional certifications such as BCPS for Infectious Disease or BCOP for Oncology are also recognized.
  • Current ACLS/ALS certification within 30 days of hire, preferably from American Heart Association or American Red Cross.
Licenses and Certifications
  • Valid Pharmacist license in the applicable state.
  • Board certification and antimicrobial stewardship training or certification if applicable.
  • American Heart Association Basic Life Support (BLS) preferred.
Preferred Qualifications
  • Three to five years of pharmacy experience in a hospital setting.
Knowledge, Skills, and Abilities
  • Strong understanding of federal and state pharmacy laws and regulations.
  • Proficient computer skills.
  • Excellent communication abilities.
Physical Requirements
This position involves medium work, occasionally exerting up to 50 pounds and frequently up to 10 pounds. Approximately 26-50% of the workday may involve standing or walking.
