Join a dedicated team providing direct mental health services to preschool-aged children facing significant challenges in the Midwest region. As an Advanced Case Manager, you will play a vital role in delivering community-based, interactive care designed to enhance overall functioning and supportchildren in the least restrictive environments possible.
Key Responsibilities
- Deliver skill training aligned with measurable goals determined by a Qualified Mental Health Professional.
- Provide intensive, strengths-based mental health services within the community.
- Collaborate with families and treatment teams to develop and maintain client-centered treatment plans.
- Support youth in understanding their symptoms and exploring solutions.
- Facilitate connections with natural community supports and coordinate mental and physical healthcare services.
- Offer crisis stabilization and conduct trauma-informed risk assessments under professional guidance.
- Work closely with community partners and caregivers to ensure successful reintegration of clients.
- Complete all required documentation accurately and promptly.
Schedule Monday through Thursday, 10-hour shifts. Education and Experience - Required: Bachelor's degree in Social Work, Psychology, or related field; OR Associate's degree plus 2+ years of relevant experience; OR equivalent combination of education and experience totaling 4+ years.
- At least one year of experience working with children.
- Preferred: Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
- Three to five years of professional experience in mental health or related services is preferred.
Licensure and Training - Completion of CPST Case Management online training and other required screenings within 60 days of employment.
- Valid driver's license, insurance, and reliable vehicle required.
