Details Care setting: Home Health Monday–Friday, day shift (8am–5pm) 8-hour shifts Position involves approximately 25% travel EMR: Cerner 13-week assignment Requirements Minimum 2 years of physical therapy experience required Active CA PT license required (license required if applicable); pending licenses will not be accepted Active BLS certification (AHA or ARC) required; pending certifications will not be accepted 2 professional references required: one supervisor from within the last 12 months, and one peer or supervisor from within the last 3 years, including dates of employment and eligibility for rehire Copy of driver's license required for consideration Additional Information Candidates may not have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity facility within the past year; candidates who worked PRN through outside staffing firms will be considered American Traveler Job ID P-711293.
