Mental Health Intensive Case Management (MHICM) Program

Major Duties and Responsibilities

The clinical and program activities assumed under this position are conducted as part of the Mental Health Intensive Case Management (MHICM) Program. The goal of the MHICMProgram is to develop, implement and assess the benefit of a behaviorally oriented program or psychosocial rehabilitation, intensive case management and community-based treatment for chronically mentally ill veterans who have been high uses of Inpatient Mental Health services. Work is done in the context of an interdisciplinary treatment team, and extensive liaison activities with other interdisciplinary teams are required to provide continuity of care for assigned veterans in outpatient settings.

• Provide a full range of therapeutic interventions in units/programs in the Hampton VA Health Care System to which they are assigned and has clinical privileges to practice including diagnostic assessment, medication evaluation and management. Recognizes the need for the appropriate level of care including routine, urgent and emergent status and recognizes the need to refer the patients for inpatient care.
• Will provide support and clinical oversight to their program.
• Will work with interdisciplinary team. Participates effectively in team meeting and treatment planning conferences and collaborates with multidisciplinary team members in a manner that enhances coordination of comprehensive patient care.
• Performs appropriate evaluation and management of patients to include ordering of diagnostic tests and consultations and discriminates between normal and abnormal findings and makes appropriate treatment decisions.
• Provides a full range of therapeutic interventions to units/programs to which assigned and has clinical privileges to practice in.
• Is involved in the supervision and teaching of house staff and other trainees assigned to the clinical area as appropriate.

Telework: Offered on an AD-HOC Basis to support the needs of the facility. Telework is generally not offered on a regular and re-occurring basis.
