What You'll Do As a Home Health Speech-Language Pathologist, you'll help patients maximize their communication, cognition, and swallowing function through individualized therapy plans designed around their personal goals. Your responsibilities will include:
Evaluating patients with speech, language, cognitive, voice, and swallowing disorders.
Developing individualized treatment plans that support functional independence and improved quality of life.
Providing evidence-based therapy in the comfort of patients' homes.
Monitoring patient progress and modifying treatment plans as appropriate.
Educating patients and caregivers on therapy techniques, home exercises, and strategies for continued success.
Collaborating with physicians, nurses, physical therapists, occupational therapists, and other healthcare professionals to coordinate exceptional patient care.
Completing accurate and timely clinical documentation.
Qualifications
Master's or Doctoral degree in Speech-Language Pathology
Current New Hampshire Speech-Language Pathologist (SLP) license
Minimum of one (1) year of speech therapy experience preferred
Home health experience is welcomed but not required-we're happy to support therapists transitioning into home health
Current CPR Certification
Strong clinical assessment, communication, and interpersonal skills
Compassionate, patient-centered approach with excellent organizational skills
Knowledge of state and federal home health regulations preferred
