Contract | 13-Week Assignment | Skilled Nursing Facility (SNF)
Key Responsibilities
  • Evaluatepatients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
  • Deliver direct patient care focused on improving mobility, strength, and functional independence
  • Document patient progress, treatment sessions, and outcomes in accordance with facility and regulatory standards
  • Collaborate with interdisciplinary care teams to coordinate patient goals and discharge planning
  • Educate patients and families on therapeutic exercises and home safety strategies
Required Qualifications
  • Active Physical Therapist (PT) license in the state of Idaho or eligibility for licensure
  • Graduate of an accredited Physical Therapy program
  • Current CPR/BLS certification
  • Experience working in a skilled nursing facility or post-acute care environment
Preferred Experience / Skills
  • Prior travel or contract therapy experience
  • Familiarity with MDS documentation and SNF regulatory requirements
