Contract | 13-Week Assignment | Skilled Nursing Facility (SNF)
Key Responsibilities
Evaluatepatients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
Deliver direct patient care focused on improving mobility, strength, and functional independence
Document patient progress, treatment sessions, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to coordinate patient goals and discharge planning
Educate patients and families on therapeutic exercises and home safety strategies
Required Qualifications
Active Physical Therapist (PT) license in the state of Idaho or eligibility for licensure
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Experience working in a skilled nursing facility or post-acute care environment
Preferred Experience / Skills
Prior travel or contract therapy experience
Familiarity with MDS documentation and SNF regulatory requirements
