DescriptionSIGN-ON BONUSJOB PURPOSE: The Physical Therapist provides physical therapy services to patients in the homesetting. Work includes responsibility for the assessment, planning, implementation,and evaluation of patient's physical therapy plan of treatment. Salary listed is for a full time position.$10,000.00 Sign On Bonus!QUALIFICATIONSMinimum of one year practicing as a Physical Therapist preferred.Possess and maintain a current license as a Physical Therapist from the MD State Board of Physical Therapy Examiners.RESPONSIBILITIESConducts a complete physical evaluation in order to determine appropriate treatment in accordance with the functional level of the patient.Plans, develops, implements, and evaluates patients for appropriate PT services.Completes a plan of care that is thorough with measurable short and long-term goals.Provides physical therapy interventions and procedures according to the MD State Practice Act, to manage patient functional limitation and provide instruction to patients and families to manage functional deficits/limitations.Assist and instruct patient and/or caregivers in obtaining and using appropriate equipment.Teaches home physical therapy programs to patients and/or their families and assists them in planning/implementing equipment needs for home use.Attends case conferences in order to assure interdisciplinary communication regarding patient status.Observes infection control precautions in order to prevent spread of disease.Attends staff meetings and in-services as required.Performs additional activities as assigned.RequirementsSPECIAL REQUIREMENTSThe Full Time or Part Time Physical Therapist will work a rotating Saturday schedule.Possess and maintain documentation of freedom of TB in a communicable form.Possess and maintain documentation of annual influenza vaccination.Mandatory Competencies as defined by the agency Education Plan.Maintain current CPR certification.Maintain current driver's license.Maintain current auto insurance of at least $100,000 total liability insurance per person and $200,000 total liability insurance per accident.POSITIONS SUPERVISEDPhysical Therapy AssistantPHYSICAL DEMANDSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. Thisposition is very active and requires standing, walking, bending, kneeling and stooping all day. The Physical Therapist 2employee must frequently lift or move items weighing over 20 pounds. This position requires the ability to transfer, lift, move, and turn patients to and from bed, wheelchair, toilet, tub, shower, and occasionally the floor.WORKING CONDITIONSWork is performed in the patient's home. Exposure Risk to Blood borne Pathogens: Category A - routine exposure to blood and body fluids. Occasional exposure to hazardous materials and infectious diseases.Extensive travel is required, sometimes in hazardous weather conditions.EMERGENCY RESPONSIBILITYExpected to respond to emergency situations involving the safety of the patient in their homes.This includes the ability to assist with a possible evacuation of patient, or notify the proper agency/facilityin case of any type of emergency situations such as calling an ambulance or the fire department.This position description is not intended to be all-inclusive.
