Responsibilities include psychosocial evaluation, crisis intervention, psychotherapeutic support and counseling, and/or psychotherapy to patients and their families. Act as an advocate, assisting assigned patients in physical, emotional, and social adjustment, and facilitate individualized care plans within a multidisciplinary team.Maintain continuity of care by developing and implementing patient discharge plans from admission onward, collaborating with patients, families, guardians, caseworkers, and community resources. Provide ongoing monitoring, individual and group counseling, and facilitate adjustment to secondary effects of illness. Serve as liaison between facility, family, government, and community agencies, document interventions, complete required paperwork, and ensure smooth transitions of care. Respond to crisis management situations per hospital and departmental policy.
