About UsVolunteers of America is a nonprofit human services organization committed to serving people in need, strengthening families, and building communities.Job Summary and PurposeThe Resident Care Manager I advocates for residents and provides intensive case management services, meeting participants at least weekly. The manager is the primary point of contact to coordinate services and care for assigned clients, handling a caseload of individuals in need of a wide variety of human services resources.Duties and ResponsibilitiesConduct comprehensive screenings and assessments with individuals or families to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information.Assess barriers, develop a case plan, and coordinate individualized planning to meet short‑ and long‑term needs.Provide service linkages and support systems to ensure identified needs are met.Coordinate case conferences, consult with service teams, monitor progress of clients on case plans, and assist clients to meet agreed goals.Act as an advocate for clients before judicial, community, social service, and administrative bodies; facilitate support groups and client participation as appropriate.Coordinate participants' care in collaboration with the entire care team, including primary care physicians, mental health providers, substance use disorder providers, and housing navigators.Work with Community Property Management to provide service linkages and support systems to residents.Maintain daily contact to identify new and ongoing service needs.Coordinate access to supportive services, including health services, medical appointments, and crisis intervention as needed.Essential DutiesServices for this position must be provided on‑site; the employee must report to work on-site each day of active employment.Conduct regular individual meetings and develop detailed plans of action to meet client needs.Coach and provide social support by accompanying participants to activities and services.Coordinate case management with the team, connecting participants to appropriate services.Advocate for participants before agencies and community settings.Build capacity and motivate participant progress.Coordinate the participant's care in collaboration with the entire care team.Perform other duties as required and subject to change.RequirementsMust pass a fingerprint clearance, background check, including criminal history, personal references, and employment and education verifications.Ability to perform under potential emotional stress and conflicts, dealing with difficult, uncooperative, and potentially aggressive clients.EducationBachelor's degree in Social Work, Mental Health, or a related discipline.OR two (2) years of experience providing direct mental health services or ICMS.ExperienceMinimum one (1) year of experience working with homeless individuals.Experience working with participants with mental illness, chronic health issues, and substance use disorders.Preferred QualificationsBachelor's degree in Social Work, Social Services, Public Administration, Public Health, or a related field.HMIS navigation experience.Volunteers of America is an Equal Opportunity Employer.
