What You Will DoProvide personal care and assist with ADLs according to the Plan of Care.Ensure a safe environment for patients.Safely ambulate or transfer patients as appropriate.Assist with passive and active ROM exercises as directed by the nurse.Prepare nutritional snacks as appropriate for patients.Under the supervision of a nurse, assist patients with pre‑poured medications that are ordinarily self‑administered and prescribed by a physician under nursing supervision (Home Care only).Under the direction of a nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.Recognize and promptly report any changes in a patient's mental or physical status to the nurse.Record activities provided and observations of patients in the clinical record or appropriate documents.Complete charting, timesheets, and mileage logs as defined by agency policy and procedures.Attend and actively contribute during patient care conferences.Participate in HNA discipline meetings, team meetings, staff meetings, retreats, and in‑services as appropriate.Attend in‑services to obtain three education hours per quarter.Participate in the on‑call rotation as scheduled (Home Care only).Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.).Demonstrate positive customer‑service responsiveness according to agency standards.Be flexible in meeting patient/family needs that require variable shifts and weekends.Participate in chart review quality improvement activities and other meetings as assigned.Participate in coordination and communication with volunteer staff.Provide basic caregiving education to caregiver/family members such as turning and positioning, transfer techniques, and changing linens.Maintain transmission‑based precautions.Complete annual competency requirements.Success CapabilitiesMust be able to read and write English proficiently.Must be a state‑tested nursing assistant in good standing.Equivalent education or experience may substitute for minimum qualifications except when there are legal requirements such as a license, certification, or registration.DetailsTotal rewards package to include retirement, health, dental, vision, voluntary benefits, and corporate discounts.Tuition assistance.Non‑exempt role.Protocols in place for wellbeing during COVID‑19.Conditions of EmploymentCompliance with the annual flu‑shot policy or ability to provide exemption documentation.Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.Provide an active driver's license.Ability to provide proof of eligibility to work in the United States.High level of integrity, ethics, and professionalism.NoteThe responsibilities of this position are described above, and they may be subject to change at any time due to reasonable accommodations or other reasons.
