DescriptionThe Puerto Rican Cultural Center was founded in 1973, The Puerto Rican Cultural Center (PRCC) is a non-profit, community-based umbrella institution that seeks to serve the socio-cultural needs of Chicago's Puerto Rican and Latino community. It is built on the following principles: a philosophy of self-determination, a methodology of self-actualization and critical thought, and an ethics of self-reliance best expressed in the motto, ‘To live and help to live.'Position SummaryThe Case Manager supports youth at high risk of violence involvement by providing culturally responsive, trauma-informed case management services. This role focuses on building trust, connecting youth to resources, and supporting their progress toward personal, educational, and career goals.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.QualificationsBachelor's degree in Social Work or a related fieldValid driver's license and access to a reliable vehicleStrong commitment to working with youth and seeing tasks through to completionExcellent communication, outreach, and relationship-building skillsKey ResponsibilitiesProvide individualized, youth-centered case management and goal planningBuild trusting relationships and engage youth through proactive outreach strategiesConduct intake, screening, and assessments to determine eligibility and service needsDevelop and implement case plans using a strength-based, healing-centered approachConnect youth to services, including education, employment, housing, and mental health resourcesTransport youth to appointments and provide transportation support as neededAttend court appointments and assist with advocacy and support servicesMaintain accurate documentation and report weekly progress in designated data systemsService AreasMental health and social-emotional support (including CBT-informed activities)Education (school re-engagement, GED, post-secondary enrollment)Employment and job readiness programsHealth and wellness servicesHousing and basic needs supportAccess to public benefits and other supportive servicesEngagement & PartnershipsEngage and recruit youth through community outreach and referrals from schools, courts, foster care systems, shelters, law enforcement, and community-based organizations.
