Physical Therapist. Reports To: Clinical Supervisor / Nursing Supervisor.ResponsibilitiesProvide physical therapy services to patients according to a written individualized plan of care and within the scope of the Physical Therapy Practice Act.Assess and evaluate therapeutic, rehabilitative, and functional status; evaluate home environment; assess muscle strength, mobility, gait, range of motion, and transfer capabilities.Direct physical therapy treatment and provide equipment to increase patient function and independence.Instruct patients, families, and caregivers on the use and care of therapeutic appliances.Determine priority needs for physical therapy; report to physicians on patient reactions to treatment or changes in condition.Initiate physical therapy programs and instruct personnel and family/caregivers in phases of therapy, including case conference participation.Educate patients on prosthetic device use.Prepare and submit clinical and progress summaries and participate in patient discharge planning.Provide physical therapy consultation to families and caregivers as appropriate.Provide education programs for nursing personnel and participate in interdisciplinary groups.Actively participate in quality assessment, performance improvement teams, and orientation programs for hospice team members.Supervise physical therapy assistants and hospice aides when appropriate.QualificationsPossess a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.Licensed to practice as a physical therapist in California.At least two years of appropriate experience as a physical therapist (community or hospice experience preferred).Demonstrated verbal and written communication and organization skills.Licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.Current CPR certification.Understanding of hospice philosophy with focus on comfort and maintaining care at the patient's residence whenever possible.#J-18808-Ljbffr
