Tender Care LLC is a DDD service provider. We are looking for a Licensed Practical Nurse that has excellent experience with clients with developmental disabilities to support clients near Succasunna County , NJ. Our residential home serves a total of four individuals with medical needs.Shifts AvailableM-F 11pm-7am (+ every other weekend), Full-Time, Part-Time and Per DiemPay Rate$34-$37/hourQualificationsGraduated from an accredited Licensed Practical Nursing programCurrent State of New Jersey LPN licenseMinimum of one year of experience providing nursing services to individuals with developmental disabilitiesMust have experience with Bolus feedings, G-Tubes, and Suprapubic CathetersMust have a driver's license valid in the state of New Jersey and be able to commute to Dover, NJEssential ResponsibilitiesProvide prescribed treatment and health care procedures as ordered by a physicianAdvises supervisor of all concerns and/or issues related to the provision of nursing servicesCreate and distribute Individual Service Plan Medical/Health Reports detailing participant's general health, health care goals, and physician recommendations for each participantDocument on the individual's health status for various support people including family, guardian, staff, employment and other professional staff as necessaryRecords a detailed report of care given which reflects the therapeutic plan and specific aspects of the patient care plan on individual's official electronic health recordMonitor the consumer's use of medications through Medication Administration logsMake referrals for needed medical services and confirming that follow-up care is pursuedInstruct all new support staff on consumer specific medical needsProvide quarterly in-service training for support staff and ManagersMonitor weight gain or loss and other health conditions of participantsEnsure MAR's are being followed through regular reviews and unannounced checksMaintain health services supplies inventory and places supply orders when the inventory is lowSubmit IR's and UIR's for medication errors or other medical concerns with assistance from ManagersProvide medical training to staff as neededPerform other job related duties as assignedBenefits401(k)Health insurance: medical, dental, visionPaid time off#J-18808-Ljbffr
