Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 07/20/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Physical Therapist – Home Health Smyrna Health & Wellness Center | Smyrna, DE Assignment Overview Position: Physical Therapist (Home Health) Facility: Smyrna Health & Wellness Center Location: Smyrna, Delaware Assignment Type: Travel Contract Length: 13 Weeks Start Date: ASAP (Scheduled Orientation) Schedule: Monday – Friday | 8-Hour Day Shifts Guaranteed Hours: 40 Hours per Week Pay Frequency: Weekly On-Call/Weekend Requirement: To be discussed during the interview Requirements Active Delaware Physical Therapist License required. Home Health experience required. OASIS documentation experience required. Candidates within a 50-mile radius will be considered local. Coverage Area Physical Therapists are primarily assigned to one of the following: Sussex County area Therapists generally remain within their assigned region but may occasionally travel to neighboring areas based on patient care needs. Daily Productivity Expectations Standard 8-hour workday , including patient documentation. Typical daily caseload includes: 6 patient revisits (with approximately 1 evaluation ), or 4–5 revisits plus 1 new admission/evaluation Talent Burst Job ID 26-18783.
