Job Description Job Summary: TRAVEL - RN - Case Manager provides comprehensive case management for acute care patients across step-down units, partnering closely with clinical teams to support safe transitions of care. This role focuses on timely assessments and coordinated discharge planning in a fast-paced environment. Responsibilities:
Conduct patient case management assessments to identify needs, risks, and care coordination priorities.
Coordinate discharge planning with physicians, mid-level providers, and interdisciplinary teams.
Utilize EMR systems including Meditech and Midas to document care and workflow activities.
Support care transitions by arranging appropriate post-acute resources and follow-up requirements.
Collaborate with patient care teams to ensure continuity of care throughout the hospitalization process.
Communicate effectively with stakeholders to maintain an accurate and up-to-date plan of care.
Qualifications:
Maintain at least 1 year of current acute care Case Manager RN experience.
Hold an active RN license in the state of Kansas or meet RN/Licensed social worker credential requirements.
Possess a current BLS certification.
Demonstrate Meditech and Midas experience or equivalent EMR case management proficiency.
Show strong ability to thrive in a flexible, high-volume, fast-changing environment.
Prefer experience with Midas and HCA case management and prior HCA hospital experience.
