Housing Case Manager (Licensed Housing)Location: Elmira, NYEmployment Type: Full-TimeSalary: $21.50 per hourGeneral DescriptionUnder the supervision of the Senior Housing Supervisor (SHS), the Housing Case Manager (HCM) coordinates care for resident services. The HCM provides education, support, and accountability to ensure residents receive needed services and program compliance with contractual obligations.The HCM offers direct services to residents: prepares assessments, develops and implements individual service plans, facilitates groups and activities, teaches and supports residents in developing ADA skills, communication and self-advocacy skills, facilitates resident involvement and community activities, makes referrals to community-based services, assists in accessing and maintaining entitlements and benefits, advocates for needed services, and assists residents in meeting tenancy obligations.Essential Duties and ResponsibilitiesMeets regularly with the Senior Housing Supervisor for review of file documentation, ensuring files comply with funding regulations.Participates in agency orientation on program operations, policy, procedure, goals, and objectives.Facilitates integration of residents into the community using community-based services when possible.Maintains accurate case file records and reports according to each funding source/program requirements.Documents assessments, service plan goals, reviews, utilization reviews, hospitalizations, discharge planning, progress notes, and income as required.Submits resident files to the SHS within 28 days of admission for audit and review.Renews file documents with expiration dates, including Release of Information, consents, Physician authorizations, Housing Quality Inspection packets, Tenant Calculation Worksheets, and Income Verification.Ensures licensed apartments meet recertification standards and reports maintenance needs.Ensures resident fees, rent and phone bills are paid timely; assists with delinquency issues.Assists residents in complying with treatment/service providers to avoid lapses in service/funding.Acts as advocate/mediator for residents experiencing landlord/tenant disputes or provider, employment, funding issues.Supports residents transitioning to a more or less restrictive level of care, maintaining continuity of care with providers and funding streams.Provides a resource to ensure resident access to services and protection of rights/confidentiality.Conducts routine housing inspections to identify resident needs and communicate concerns with the SHS, working with residents to maintain unit or access needed services.Attends staff meetings, treatment team/provider meetings, and staff development/training seminars per agency policy.Completes other duties as assigned.QualificationsAssociate's degree in Human Services or related field preferred.Two years' experience in Human Services or related field preferred.Relevant combination of education and experience will be considered.Physical Demands/Work EnvironmentThe physical demands include use of hands to manipulate items, standing, sitting, walking, bending, stooping, kneeling, reaching, climbing stairs, lifting or moving objects up to 10 lbs, and prolonged desk or vehicle time.
