Facilitate the integration of new participants into the health care delivery system including meds, immunizations, routine monitoring of chronic problems, and foot & wound care
Administer and monitor participant meds prescribed by the primary care providers and reconcile end of month MAR
Sign for medications delivered by the pharmacy daily; request refills of meds in a timely manner.
Add new meds to the clinic med cart or refrigerator
Remove & dispose of discontinued meds
What We're Looking For
Licensed Practical Nurse (LPN) with unrestricted LPN license in the state of Michigan
At least 2 years of nursing experience
Medication administration experience and vaccines administration experience
Minimum of 1 year experience working with the frail or elderly population preferred
Ability to manage multiple tasks comfortably while staying organized and professional
