We will assign you a preceptor, provide 1:1 feedback, and offer free access to earn continuing education.

We will support you in obtaining additional state licensure and credentialing in neighboring states – or others you may want to visit while completing health assessments.

You can work during the day, evening, and/or weekend based on your schedule.

Nurse Practitioner or Physician Assistant

Master's Degree or commensurate experience and satisfactory completion of NP licensure

Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed

AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty

Current BLS, ACLS or CPR certification

Valid state driver's license for the state of residence

Automobile insurance coverage requiring for the state of residence (or ability to obtain)

Bodily Injury: $100,000 per person/$300,000 per incident

Property Damage: $50,000

Collision and comprehensive deductibles: $1,000 or less

Familiarity with employee health/medical record software

1 year of NP experience preferred

Additionalcompensation may be available for qualified candidates who are fluent in languages other than English and use those language skills during member visits.

Compensation for this specialty generally ranges from $125,000 - $175,000.

Flexible scheduling is offered where providers can flex their time between 8am-8pm over the 7-day work week.

For PAs: Current NCCPA certification and state licensure

Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations

Active and unrestricted driver's license

Access to reliable transportation to complete home visits

Ability to navigate varied home environments; ability to sit/stand/kneel as needed to perform assessments

Candidates are required to pass a drug test before beginning employment.

If you're working in a state that allows NPs/PAs to practice independently (without Collaborative Agreement), you must get approval from your licensing board-if required. New hires who are eligible but haven't applied yet must do so within 1 month of starting. If you're not eligible at the time of hire, you must begin working toward eligibility within 1 month and apply for approval within 3 months of becoming eligible

1+ years of clinical experience (family, geriatric, or home health preferred)

Proficiency with electronic medical records and technology

Ability to transport equipment weighing up to 30 pounds and navigate stairs as part of home visits

Fluent in any languages besides English
