High School Diploma / GED
3+ years of experience working within a community health setting or community outreach experience
2 plus years of professional experience working with diverse multi-ethnic and socioeconomic populations.
1 plus year of field-based experience
Intermediate level of computer experience with the ability to navigate a Windows Environment, especially Microsoft Excel
Required to have a dedicated work area established that is separated from other living areas and provides information privacy
Ability to keep all company sensitive documents secure
Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Active and unrestricted driver's license and access to reliable transportation enabling you to travel to member homes and communities within a designated area
Must reside within a commutable distance to Charlotte and surrounding communities in North Carolina
Must be able to plan routes for best time management of schedule and travel time/distance
Ability to have flexibility with scheduled hours based on member's availability and/or needs
