Full Time Home Health Physical Therapist Case Manager for South East Chicago.

Normal Office Hours Monday Thru Friday 8.00 AM to 5.00 PM.

Position includes weekend coverage rotation every 4th to 5th weekend and 2 Advocate holidays per year based on Departmental Needs.

Physical Therapist (PT)

Physical Therapist. Licensed in the State of Illinois (or eligible for licensure)

A valid driver's license issued by the Division of Motor Vehicles.

Master's Degree in Physical Therapy.

Able to demonstrate compliance with state continuing education requirements

1 year of clinical experience or demonstration of strong spectrum of clinical affiliation experience

Basic knowledge of computer use (i.e. Microsoft Word, email access and use)

Proficient use of all home based therapy equipment and assistive devices, (balls, bands, weights, Hoyer lifts, etc)

Ability to work rotating weekends and holidays.

Ability to attend mandatory informational/education/branch meetings.

Requires full range of body motion including: a. Bending, sitting and standing b. Climbing stairs while carrying objects c. Pulling, pushing, reaching, carrying and lifting; (must be able to lift/carry at minimum 25 pounds)

Ability to lift up to 35 pounds without assistance.

Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required

Ability to learn to use computer based scheduling and documentation system
