Current State of Illinois Physical Therapist licensure
A valid driver's license is required if the incumbent is selected to perform related duties at an offsite location
The incumbent must maintain automobile liability coverage as required by law
American Heart Association BLS CPR certification
For Physical Therapist II level - minimum of 2 years work experience required
Notification from Continental Testing Services advising of receipt and approval of application for licensure (if first-time test taker).
“Authorization to Test” (ATT) letter from Federation of State Boards of Physical Therapy
Confirmation of Computer-Based Exam
Current CPR Certification issued by the American Heart Association BLS provider
