Graduate of a Physical Therapist Assistant curriculum

A active PTA license in Kentucky.

Minimum of one-year experience working with a frail or elderly population required

Good working knowledge of physical, psychosocial and behavioral needs of the older population.

Working knowledge of practice boundaries for Physical Therapist and Physical Therapy Assistant (P.T.A.).

Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

Must meet a standardized set of competencies for the specific position description established by Senior CommUnity Care and in accordance with CMS before working independently.

With a Monday–Friday, 8:00 am–5:00 pm schedule

Nurse Practitioner / Physician Assistant

Master's degree from accredited nursing program.

Active Ohio APRN license with prescriptive authority

Active Board Certification in appropriate nursing specialty.

Three (3) years of nursing experience.

Master's degree from accredited physician assistant program.

Active Ohio Physician Assistant license with prescriptive authority

Certification by the National Commission on Certification of Physician Assistants.

Minimum of one (1) year experience is required.
