Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program

Active and unrestricted Nurse Practitioner license in the state which you reside, or ability to obtain by start date

Graduate of an accredited Physician Assistant degree program

Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, remain in a stationary position and position self to perform physical assessment

Driver's license and access to reliable transportation

Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice) (SCC04)

1+ years of clinical experience in practice (long-term care setting preferred)

Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs

Understanding of Geriatrics and Chronic Illness

Understanding of Advanced Illness and end of life discussions

Proficient computer skills including the ability to document medical information with written and electronic medical records

The base hourly range for this role is $59.10 to $65.50**** per hour (plus evening and weekend differential)

Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.

Family Nurse Practitioner

Completed an accredited FNP program with a national certification

In the past 5 years, practiced as an Advanced Practitioner for at least: * 2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)

Currently licensed in one of these states : CA, OR, WA, AZ, or CO with ability to obtain additional state licenses

Oregon licensed with a Psychiatric Mental Health Nurse Practitioner (PMHNP) certification.

Active DEA.

Experienced in working with adults, children & adolescents, or across the lifespan.

Experienced in both medication management and psychotherapy.

Must reside within commutable distance of local clinic(s).
