Board certification in relevant practice area (e.g., BCCP, BCACP, BCPS) – Required or must be eligible for and obtain within eighteen (18) months of hire
Graduate Degree – Graduate of an accredited School of Pharmacy – Minimum
Completion of a PGY1 ASHP accredited residency program (or equivalent work experience)
Completion of a PGY2 ASHP accredited residency program in ambulatory care – Preferred
Professional certificate in relevant practice area(s) – Preferred
