Active and unrestricted New York State Social Work license (LMSW or LCSW), as required.
Minimum 2 years of experience in care management, case management, managed care, behavioral health, long-term care, home care, or a related healthcare setting.
Knowledge of New York Medicaid, Medicare, Managed Long Term Care (MLTC), and Long-Term Services and Supports (LTSS).
Experience conducting psychosocial assessments and developing person-centered care plans.
Proficiency in Microsoft Office applications and electronic documentation systems.
Ability to travel within the assigned service area to conduct member visits as needed.
Master's Degree in Social Work (MSW) from an accredited college or university required.
