High School Diploma/GED
3+ years of experience working within a community health setting or community outreach experience
2+ years of professional experience working with diverse multi-ethnic and socioeconomic populations
1+ years of field-based experience
Intermediate level of computer proficiency with the ability to navigate a Windows Environment, especially Microsoft Excel
Required to have a dedicated work area established that is separated from other living areas and provides information privacy
Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Ability to keep all company sensitive documents secure
Ability to plan routes for best time management of schedule and travel time/distance
Ability to have flexibility with scheduled hours based on member's availability and/or needs
Reside within a commutable distance to Greenville, Anderson and Greer, and surrounding communities in South Carolina
Access to reliable transportation
Valid US driver's license enabling you to travel to member homes and communities within a designated area
