Occupational Therapist

Bachelor's, Master's, Doctoral degree from an accredited college or university in the area of Occupational Therapy is required.

Licensed as an Occupational Therapist by the State Board of Occupational Therapy Examiners of Texas.

BLS certification is required.

Must also successfully complete the pre-employment/post job offer health screening examination and the annual screening, each year thereafter as an employee.

Master's Degree in Nursing (MSN) and/or master's degree in Physician Assistant Studies (MMS)

Masters Degree in Nursing

Level 1: New Grad with up to 1 years of experience as a nurse practitioner; new grad Physician's Assistant with up to 1 years of experience as a physician's assistant

Level 2: Nurse practitioner with over 1 years of experience and less than 5 years of experience; Physician's assistant with over 1 years of experience and less than 5 years of experience as a physician's assistant.

Level 3: Nurse practitioner with over 5 years of experience and less than 10 years of experience; Physician's assistant with over 5 years of experience and less than 10 years of experience as a physician's assistant.

Level 4: Nurse practitioner with over 10 years of experience; Physician's assistant with over 10 years of experience as a physician's assistant.

License and DEA must be active, in good standing, and verifiable with the proper regulatory agency.

DOT certification is required to be obtained by FT and PT Providers within 120 days of employment and maintained during employment.
