The hourly pay for this role will range from $20.38 to $36.44 per hour based on full-time employment.

Monday - Friday with some evening and weekend hours to attend community events

Ability to work Monday through Friday, 8 AM to 5 PM and open to working some evenings and weekend hours for events

High School Diploma/GED (or higher)

1+ years of experience in local community-based resources

Intermediate level of proficiency in MS Word and Outlook

Designated workspace inside the home with access to high-speed internet

Ability to travel in assigned territory approximately 50-75% of the time

Resident of Washington state

Bilingual Spanish and English

CHW Accreditation

2+ years of community outreach work experience

2+ years of field-based experience

Knowledge and/or experience with behavioral health or substance use disorders

Care management experience

Experience working in Managed Care

Experience with EHR

Knowledge of Medicaid and / or Medicare population
