High School Diploma/GED
3+ years of experience working within a community health setting or community outreach experience
2+ years of professional experience working with diverse multi-ethnic and socioeconomic populations
1+ years of field-based experience
Intermediate level of computer experience with the ability to navigate a Windows Environment, especially Microsoft Excel
Ability to keep all company sensitive documents secure
Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Required to have a dedicated work area established that is separated from other living areas and provides information privacy
Active and unrestricted driver's license
Access to reliable transportation enabling you to travel to member homes and communities within a designated area
Must reside within a commutable distance to Milwaukee, and surrounding communities in Wisconsin
Ability to have flexibility with scheduled hours based on member's availability and/or needs
Ability to rent a car if exceeding 100 miles per day
